3M Health Information Systems compiles and maintains many data elements related to medical necessity. Below are possible file formats that can be created.

Basic Crosswalk Policy File
Compiled from a collection of all national Local Medical Review Policies, this contains basic CPT-to-ICD crosswalk with optional effective date.

Carrier's specific Policies File
The Medical Necessity dictionary file lists the ICD-9 and CPT code pair per specific Medicare LMRP  carrier / Fiscal Intermediary. We support Part A and Part B LMRP's of all 50 states. The file is updated on a monthly basis and capturing carrier's policy changes and news letters updates.

NCD - National Coverage Determinations
National Coverage Determinations (NCD) effective November 25 2002, are nationwide policy statements that describe the clinical circumstances for which a service is considered to be reasonable and necessary.

DME LMRP
The DME LMRP file lists the ICD-9 and CPT code pair per specific Medicare DME polices for all DME regions. The file is updated on a monthly basis and capturing carrier's policy changes and news letters updates.
